Study of the Independence IBOT 3000 Mobility System: an innovative power mobility device, during use in community environments

OBJECTIVE: To test the safe and effective use of a new mobility device, the Independence IBOT 3000 Mobility System, handicap scooters and medical scooters, by people with a disability. DESIGN: A prospective, balanced, open-label evaluation that used participants as their own controls. SETTING: Home and community environments. PARTICIPANTS: Twenty subjects who use a mobility device. INTERVENTIONS: Subjects used the test device for up to 2 weeks in their home and community environments. MAIN OUTCOME MEASURE: Safety was determined by comparing the number of reported adverse events, including falls, in the test device compared with their own device. Effectiveness was measured by comparing scores from the Community Drive Test in the test device versus their own chair. RESULTS: No adverse event requiring medical attention was reported for either device and a similar number of device falls were reported for each device. The scored driving test results found that the test device was shown to statistically improve (P <.001) the subjects independent functional mobility skills in a community environment. Ten of 20 subjects were able climb stairs independently; the remaining 10 subjects were able to climb stairs with the assistance of only 1 person. CONCLUSIONS: People who are properly assessed, and participate and successfully complete the training, will benefit through increased independence in community mobility activities.

Older adults’ use of, and satisfaction with, electric powered indoor/outdoor wheelchairs

BACKGROUND: Research documenting the experiences of electric medical scooters indoor/outdoor medicare power wheelchairs(EPIOC) users has generally failed to take into account the specific needs and concerns of older adults. This study sought to qualitatively examine the older EPIOC users’ satisfaction with the chair and service providers. METHOD: Eight women and nine men aged between 60 and 81 (mean 69) years were recruited through a specialist wheelchair service database. All had severe mobility disabilities. Analysis was performed using a qualitative framework approach. RESULTS: Participants reported a variety of EPIOC uses, including shopping and some social contact. Moderate satisfaction with the chair was reported. Use was compromised by indoor and outdoor environmental barriers; and the chair not meeting users’ needs. Accidents were rare, but many users still experienced insecurity in the chair. High levels of satisfaction with the service were reported, although concern was expressed over length of waiting times. CONCLUSIONS: EPIOCs proved useful to most older people with disabilities. However, even those who were satisfied reported only moderate use of the chair outdoors. Limited use related to an infrequent need for outings, outdoor barriers, feelings of insecurity over chair safety and lengthy waiting times for chair delivery and required modifications. Service providers should be aware that older EPIOC users may require extra support and the provision of timely adjustments to increase chair use.

Longitudinal assessment of vibrations during manual and power wheelchair driving over select sidewalk surfaces

Wheelchair users rely on their power wheelchairs for mobility for extended periods of time every day. According to the International Standards Organization 2631-1 standard on human vibration, individuals in a seated position when exposed to whole-body vibrations (WBV) are at risk of injury. This study evaluated vibration exposure during manual and power wheelchair driving over nine sidewalk surfaces and differences in vibration exposure over 3 years. Ten nondisabled subjects were asked to drive a manual wheelchair at 1 m/s and a power wheelchair at 1 m/s and 2 m/s over nine sidewalk surfaces while WBV were measured at the seat and footrest of the wheelchair. At 1 m/s, significant differences existed between surfaces and years at both the seat and the footrest for the manual and power wheelchair users. At 2 m/s, significant differences existed between surfaces and years at the seat and the footrest for handicap scooters users. Our results show that both manual and power wheelchair users may be at risk for secondary injuries from WBV when traveling over certain surfaces

Using the absorbed power method to evaluate effectiveness of vibration absorption of selected seat cushions during manual wheelchair propulsion

Although medicare power wheelchairs and handicap scooters users are frequently subjected to oscillatory and shock vibrations, little research has been conducted to assess the whole-body vibration exposure of wheelchair users. The purpose of this study was to determine if selected wheelchair cushions alter potentially harmful whole-body vibrations transferred to wheelchair users. Thirty-two participants, who use wheelchairs as their primary mode of mobility, contributed to this study. Four of the most commonly prescribed wheelchair cushions were selected. Participants were asked to propel their wheelchair over a simulated activities of daily living (ADL) course while acceleration and force data were recorded. A repeated measures ANOVA showed no significant differences between the different cushions for the total averaged absorbed power (p = 0.190), the 50 mm curb drop (p = 0.234), or the rumble strip (p = 0.143). A repeated measure ANOVA for the peak curb drop absorbed power revealed a significant difference in the cushions (p = 0.043). The cushions that were most effective in this testing appear to be the Invacare Pindot and the Varilite Solo. Not only did those cushions appear to have the lowest values much of the time but also they did not display the highest values. When comparing results from a similar study, absorbed power appears to be as effective in determining vibration effects in the time domain as the methods in the ISO 2631 Standard.

The effect of providing power mobility on body weight change

OBJECTIVE: To investigate whether the provision of power mobility would have an effect on body weight in adults who were first-time qualifiers for power mobility. DESIGN: This was a retrospective observational study of consecutive subjects, who served as their own controls. The medical scooters, handicap scooters records of 468 subjects who were approved for power mobility during a 17-mo period were reviewed. Three weight groups (12 mos before, at, and 12 mos after the power mobility evaluation) were evaluated with repeated-measures analysis of variance (ANOVA). The weight changes on subjects in different age groups (45-54, 55-64, 65-74, and >74), in geriatric vs. nongeriatric groups, and in different body mass index (BMI) groups were analyzed. RESULTS: Eighty-nine subjects met the inclusion criteria. They were obese (49.4%), and most of them were geriatric (64%). Congestive heart failure (30.34%) and chronic obstructive pulmonary disease (22.47%) were the two main presenting diagnoses. The repeated-measures ANOVA showed no significant weight change in the three studied weight groups. Similar results were seen in the age and BMI subgroups. CONCLUSIONS: There was no statistically significant weight change in adults who were first-time qualifiers and who used power mobility for 1 yr.

DESIGN: Convenience sample. SETTING: Motion analysis laboratory. PARTICIPANTS: Older manual wheelchair users (N=14; age, 68.2+/-5.2y) were tested. INTERVENTIONS: Kinematic and kinetic data were collected during manual wheelchair propulsion at a speed between 0.96 and 1.01m/s for 10 seconds and at a power output around 22.4W on a medicare power wheelchairs ergometer. MAIN OUTCOME MEASURES: Net shoulder joint moments were computed with an inverse dynamic model. The mechanical use of the forces at the pushrim and the mechanical fraction of effective force were measured during propulsion. RESULTS: Mechanical use and mechanical fraction of effective force had a positive and handicap scooters significant correlation with the net internal (P<.05) and external (P<.001) shoulder rotation moment, the net flexion (P<.05), and extension (P<.001) moment in the sagittal plane, and the net flexion (P<.001) moment in the horizontal plane. CONCLUSIONS: The results suggest that because the resultant force at the pushrim has a greater tangential component and a greater proportion of the maximal voluntary force, most of the net moments around the shoulder are higher. Thus the optimal way of propelling, from a mechanical point of view (ie, tangential), may not be advantageous for manual wheelchair users

OBJECTIVE: To determine and compare performance of pushrim-activated power-assisted wheelchairs (PAPAW) (iGLIDE, e-motion, Xtender) on national standards. DESIGN: Engineering performance and safety evaluation. SETTING: A Veteran Affairs and university-based research center. SPECIMENS: Nine PAPAWs. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Static, dynamic stability, brake effectiveness, maximum speed, acceleration, retardation, energy consumption, static, impact, and fatigue strength. RESULTS: There was no significant difference among the 3 models in forward stability. The iGLIDE was the most stable, whereas the e-motion was the least stable model in the rearward stability tests. All PAPAWs performed equally on the slopes of 3 degrees and 6 degrees in the forward and rearward directions. Braking distance was the highest for e-motion (5.64+/-0.28m) and the lowest (1.13+/-0.03m) for the iGLIDE in forward direction. The average equivalent cycles of all PAPAWs were 318,292+/-112,776.6 cycles (n=8) on the fatigue tests. All PAPAWs passed the impact and static strength tests. CONCLUSIONS: The standards of the American National Standards Institute and handicap scooters, power wheelchairs the Rehabilitation Engineering and Assistive Technology Society of North America could act as quality assurance tool for wheelchairs. A prototype power assist wheelchair that provides for obstacle detection and avoidance for those with visual impairments

BACKGROUND: Almost 10% of all individuals who are legally blind also have a mobility impairment. The majority of these individuals are dependent on others for mobility. The Smart Power Assistance Module (SPAM) for manual wheelchairs is being developed to provide independent mobility for this population. METHODS: A prototype of the SPAM has been developed using Yamaha JWII power assist hubs, sonar and infrared rangefinders, and a microprocessor. The prototype limits the user to moving straight forward, straight backward, or turning in place, and increases the resistance of the wheels based on the proximity of obstacles. The result is haptic feedback to the user regarding the environment surrounding the medicare power wheelchairs. RESULTS: The prototype has been evaluated with four blindfolded able-bodied users and one individual who is blind but not mobility impaired. For all individuals, the prototype reduced the number of collisions on a simple navigation task. CONCLUSION: The prototype demonstrates the feasibility of providing navigation assistance to manual wheelchair users and medical scooters, but several shortcomings of the system were identified to be addressed in a second generation prototype.
