Course of gross mechanical efficiency in handrim wheelchair propulsion during rehabilitation of people with spinal cord injury: a prospective cohort study

DESIGN: Subjects were tested at the start of active rehabilitation (t1), 3 months later (t2), and when discharged from inpatient rehabilitation (t3). They performed two 3-minute submaximal treadmill exercise blocks in a wheelchair. SETTING: Eight rehabilitation centers in the Netherlands. PARTICIPANTS: Ninety-two people with (in)complete paraplegia and tetraplegia. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Mechanical efficiency values were calculated for each block. The course of mechanical efficiency was investigated using test occasions (t1 -t3), completeness and level (paraplegia or tetraplegia) of the lesion, and power output as independent variables in a multilevel regression analysis. RESULTS: Mechanical efficiency significantly increased between t1 and t2 only. After adding level and completeness of the lesion and their interactions with time to the model, block 2 showed that subjects with paraplegia had a significantly higher mechanical efficiency than subjects with tetraplegia. Subjects with tetraplegia improved more between t1 and t2 . Differences in mechanical efficiency between subjects with paraplegia and tetraplegia could not be explained by differences in absolute and relative power output levels. CONCLUSIONS: Results showed a significant improvement in mechanical efficiency during the first 3 months of active rehabilitation. Subjects with paraplegia showed a higher mechanical efficiency than did subjects with tetraplegia, whereas the latter showed more improvement between t1 and t2 .

OBJECTIVE: To measure the acquisition, use and satisfaction with high technology equipment by spinal cord injured tetraplegic subjects. DESIGN AND SETTING: A questionnaire was mailed to 102 tetraplegic subjects who were hospitalised in the rehabilitation center of Kerpape (Ploemeur, France) between 1998 and 2004, and 59 subjects responded. The questionnaire asked about the use of telephones, computers, wheelchairs and environmental controls at home. RESULTS: When a piece of equipment was acquired, it was very often used. Patient satisfaction with equipment was 79.3%. Home phones and mobile phones were often used with options such as hands-free devices (78 and 59% respectively). A total of 64.4% of subjects acquired a manual wheelchair and 61% a power wheelchair. The most commonly acquired options on the power wheelchairs were the powered recline (73,7%) and tilt (71,1%) systems. All options were used but all were more desired than acquired. A total of 27.1% of subjects desired a pushrim-activated handicap scooters, but only 15.3% had acquired one; 695% of subjects had a computer. Communication was the first use for the computer (82.5%); 49.2% of subjects had acquired an environmental control system, but 20% desired one. The first reason for lack of acquisition was financial difficulties but also accessibility and information problems. The factor that influenced the acquisition and need for equipment was the degree of spinal cord injury. No other factor reduced patient satisfaction with equipment. CONCLUSION: Patients were satisfied with the equipment they acquired. But their needs, especially wheelchair options and environmental control systems, were not satisfied.

Brain-driven interfaces depend on estimation procedures to convert neural signals to inputs for prosthetic devices that can assist individuals with severe handicap scooters deficits. The brain-driven interface is described as a probabilistic relationship between neural activity and components of a prosthetic device that may take on discrete or continuous values. A new estimation procedure is developed for action potentials, and a corresponding procedure is described for field potentials and optical measurements. We test our framework against dominant approaches in an arm reaching task using simulated traces of ensemble spiking activity from primary motor cortex (MI) and a power wheelchairs navigation task using simulated traces of EEG-band power. Adaptive filtering is incorporated to demonstrate performance under neuron death and discovery. Finally, we characterize performance under model misspecification using physiologically realistic history dependence in MI spiking. These simulated results predict that the unified framework outperforms previous approaches under various conditions, in the control of position and velocity, based on trajectory and endpoint mean squared errors.

Many people are committed to their “stories” which really are excuses for not taking action. They live their life by them. I used to be one of the people committed to my stories/excuses. Believe me, it was learned behavior. I learned it from many different sources such as family, school, friends and work environments. For example, because of the influence of others at work, I always believed that it was best for me to stay home 4 work nights each week and only be out late on a work night once a week. As a result there are many things I have not done because of that limiting belief that it was “not good for me to go out too many times during the week”. One of the things I missed out on was learning Spanish, something I have wanted to do for years.
So why is it that we should be making conscious choices about which information we take in so that we are Continuously Learning?

The events that have happened in my life where I have had to get out of my comfort zone have all in some way, big or small, improved my life. There are still times where I want to wallow in my comfort zone. However, now I know the perils of staying there, so I do not stay there very long.
When I first started on my journey of building my home business, our dream was to be able to sponsor 10 children with “The Plan”. As we have learned and grown through the process our focus is now to sponsor 100 children. The learning has changed our attitude about life and money, which of course has resulted in changing our lives. The focus of our life has changed for the better.

Learning assists us in adapting to change. Think of a time when you dreaded having to learn something new. Why were you dreading it? Was it because you are not in the habit of learning? Did you believe it would be too hard? Did you not see any benefit for the learning? Getting back into the habit of learning is just like starting out on a bicycle, it takes a lot more effort to get going than it does once you have attained the desired speed. When we are learning we are involved with life, we are out with our peers or a whole new group of people.
What are new information are you feeding yourself? I realized that the fiction books I was feeding myself were simply a form of escape. Very similar to the young boy we met on the streets of Le Ciba in Honduras. He was one of the kids who were abandoned to the streets as young as the age of two. What he did to escape the reality of his own life was to sniff gasoline. After meeting him I realized that I was doing the same thing by reading too many fiction books.
After that I started feeding myself with more positive material. If this is one of your escape mechanisms and you desire to change it, you may want to switch the type of books that you read. Biographies are a great way to switch from fiction because they are stories too.
Once we have begun to change our learning habits by making conscious decisions about the information we feed ourselves on, the next step is to teach what we have learned. Teaching others is the best way to learn it even better. One of my great desires is for my family and friends to learn how not to get left behind. That is the reason I choose to write these articles, because assisting you in your learning also assists me in understanding better myself.

Upper-limb joint power and its distribution in spinal cord injured wheelchair users: steady-state self-selected speed versus maximal acceleration trials

OBJECTIVE: To compare upper-limb joint power magnitude and distribution between the shoulder, elbow, and wrist during maximal acceleration (MAC) versus steady-state, self-selected speed (SSS) manual medicare power wheelchairs propulsion. DESIGN: Cross-sectional biomechanic study. SETTING: Research university and teaching hospital. PARTICIPANTS: Volunteer sample of 13 manual wheelchair users with spinal cord injury below T1. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Propulsive joint power magnitude and fractional distribution among upper-limb joints. RESULTS: Wilcoxon signed-rank testing revealed shoulder power was larger for MAC versus SSS (median peak, 101.5W; interquartile range IQR, 74.6; median peak, 37.7W; IQR, 22.9; respectively) (P<.01). Elbow and wrist power were unchanged. Peak shoulder power fraction was larger for MAC versus SSS (median peak, 1.055; IQR, .110 vs peak, .870; IQR, .252) (P<.01). Peak elbow power fraction was smaller for MAC versus SSS (median peak, -.012; IQR, .144 vs peak, .146; IQR, .206) (P<.05). Peak wrist power fraction was smaller for MAC versus SSS (median peak, -.058; IQR, .057 vs peak, -.010; IQR, .150) (P<.05). CONCLUSIONS: Power at the shoulder was larger than at other joints. Peak shoulder joint power and power fraction was larger during MAC versus SSS propulsion. Elbow and wrist power fractions were smaller for MAC versus SSS propulsion. Higher joint power, present under MAC, may predispose manual power wheelchairs users to injury, particularly at the shoulder.

Effect of visual perception, visual function, cognition, and personality on power wheelchair use in adults

The purpose of this study was to determine to what extent visual perception, visual function, cognition, and personality traits affect medicare power wheelchairs use in adults. It also proposes to establish baseline information to help clinicians determine or predict power wheelchair driving performance and to develop service plans to address those driving skills that need improvement or compensation. Sixty-two adult power wheelchairs users were recruited. Standardized instruments were used to evaluate visual perceptual skills, visual function, cognitive skills, and personality traits. The results of these evaluations were then correlated with participants’ scores on a power wheelchair performance test. Strong correlations were found between power wheelchair driving performance and visual perception (p = .000), ocular motor function (p = .000 and p < or = .001), stereodepth perception (p < or = .001), and alertness to the environment (p < or = .001). No significant correlations were found between personality traits and power wheelchair driving performance. These results indicate that good visual perceptual skills, visual function, and various aspects of cognition are necessary for proficient power wheelchair use. These data will assist clinicians in identifying significant factors to consider when evaluating and training clients for power wheelchair use.
